This is mine, that was accepted. I got it awhile back from someone on here (remember, you don't "ask" for an exemption, you "declare" it:
I, _______, declare religious exemption from the COVID-19 vaccination mandate. Under Title VII of the Civil Rights Act of 1964, I attest to having sincerely held personal religious beliefs which will not permit me to abide by this mandate.
Additionally, I am not willing to describe these personal religious beliefs for the reasonable fear that if others have knowledge of them it may engender discrimination against me and/or my family.
I am aware that reasonable accommodations may have to be made regarding this declaration of religious exemption, and am willing to discuss accommodations that would be necessary and reasonable in order to come to an agreement.
Finally, I expect this letter to remain confidential, and its contents be disclosed on to those for whom there is an express need.

I have worked at Penn State University Office of Physical for 1 year of wage payroll and now 19 years full time. I wish to maintain this employment. I love what I do and have received many performance award citations. I am stating my sincerely held religious beliefs that this experimental injection(s) (vaccine) is in conflict with those beliefs. As such to be exempted from this experimental injection. I have become aware of processes in conflict with my beliefs used to create said vaccine. The use of fetal tissues or cells to create it or inject it into my body is an affront not only to my faith but to humanity and my Sincere religious beliefs in general. I am made by god and my teaching are such that Gods work should not be modified through experimental means. I have never received a flu shot in my life. From here forward I will be referring to the vaccine as; experimental injection(s).
I have answered other questions in this exemption request provided by the Affirmative Action Office. No question on this form should negate my sincerely held religious beliefs pertaining to this experimental injection. Some questions on this form are left as true or false without the ability for explanation. Other vaccines I may have taken in my life were without my consent or clear understanding the processes or ingredients comprised in those vaccines. We were all kids once and had no religious choice at the time. I will exercise those rights now that I am of age and understand the consequences with this experimental injection(s) by rejecting them and any other experimental injection(s) forward fetal tissues, cells, or not."

I got one cleared, the key is to emphasize "these are my sincerely held religious beliefs" write it a few times but don't go overboard of course. Sincerely held religious beliefs ARE legally binding as worded. Of course state some, I used the fact that the vaccines contain, or in the process of making them aborted fetus cells. That goes against my SINCERELY HELD RELIGIOUS BELIEFS.
I encourage that it be self created. If this were an honest process, any submission would qualify, but it is company by company who is going to determine if the employee is religious enough.
YES please make them your own words, anything today copied will be found out as plagiarism. Some of these people have nothing better to do than get an ego kick by finding out someone plagiarized a work. And of course any person who denies a sincerely held religious belief is in violation of discrimination laws. They CAN be sued, if denied one should file with their State Affirmative action office to start and document the process. Of course that doesn't keep one from being illegally fired for the time being but at least in the end you could be awarded damages. Could be a lengthy long process and very draining physically and emotionally. I would hope and pray for the best but prepare for the worst.
