That's actually what his ear looks like from the right angle, with some hair covering it and scrunched a little because of the mask strap - Biden has super-weird ears.
The grey hair and the pressure from the mask strap makes Biden's weird skin flap look like it stops, but it continues and joins with the inside of the top of his ear.
At the right angle and when covered with hair and when his ear is scrunched slightly from the mask straps, yes it does look like a rectangular tab.
Its a rectangle tab, i agree he has a weird tab on his ear, but this is too unnatural.
I agree its less about the tab but if that was his ear as shown in u/Dempsey examples then the skin tone would match the ear not the face as the "tab" does in his examples
I think it's just the camera angle. making it look flat when it's concave. The gray hair covering the top of the "flap" makes it look like it's rectangular the top of it is just hidden by hair and scrunched a bit by the mask straps.
If the cameraman moved left or right a few inches, the illusion would be lost.
There’s no way it’s a trick of the camera, even the skin color between his ear lobe and that rectangular shape are of a different color.
The picture might be a little low resolution on the right with the close up, but you can almost see what looks like the seam of a face mask covered by makeup, going down his jawline, along the lower black strap of the covid mask!
Would his facelift have something to do with this? That's the official story for his appearance change.
I'm not above theories of there being actors and stuff, but the facelift is a pretty smart and tough narrative to overcome.

The back of the ear is real , if you follow the tab down it’s connected to the kidney bean looking earlobe , that looks like one piece/ if you pull that tab the kidney bean shape lobe would come off with it , the hair is part of the mask , if you look at the eye it’s recessed way to deep inside the mask , kind of looking out the whole , the eyebrow u can see is fake it’s a mask , look at both eyes and eyebrows!
