WW1 and WW2 prevented the IEC from standardizing plugs when the technology was relatively new. Afterwards, countries were satisfied enough with their own plug configuration that they decided it wasn't worth it to change.

And with 120V/240V, and 50hz/60hz, it wouldn't be as simple as just replacing plugs with a new unified one.
