There's no escaping the fact that outside of metropolitan centres, gays are far less promiscuous and polysexual, because the opportunity simply isn't there, for one, and two, when you don't live in that milieu, other things take priority in life like your home, family, career, hobbies etc than sex. I'd guess that'd go for straights too in a broad general sense.
But what's missing to make infection rates so different in cities? I don't know enough or have not yet seen enough research to have a decisive opinion on that one. However, what I do know is that the epicentre of the whole AIDS thing was always SF, where hedonistic life hasn't changed much to this day.
