Most people on insulin have type 1 diabetes - not a dietary problem. The ones that are type 2 and on insulin are very likely dietary problems.
Type 1 is genetic. The immune system attacks the pancreas, which keeps it from producing insulin. Those people can't do anything to fix that other than take insulin and match it to what they eat.

You are right by the way that the most common form is mostly a dietary problem, but most people with type 2 do just fine with meds other than insulin. Type 1 - not so much.
