I'm slow, and most of you have already figured it out. Trump (and MTG) backed McCarthy so the corrupt, establishment representatives would and could not vote for him. This allowed a small number of MAGA heroes to force all of the concessions. Then, Trump came out smelling like a rose even in the mainstream media. He truly is a stable genius.

Whats the difference and between the “corrupt establishment representatives” who did not vote for McCarthy and the “small number of MAGA hero’s” (who also did not vote for him) to supposedly force the concessions? Who is who. Name names.
Establishment Democrats who'd been selected to "cross the aisles" and vote for Kevin so he became Speaker without needing to make the concessions MAGA Republicans wanted?
With MTG and DJT so openly supporting Kevin, Establishment Democrats and their Establishment Republican allies had to redo their risk/rewards calculations. Especially when Kevin openly embraced MTG and PDJTs support and the negotiations process he was engaged in with the Republican Holdouts.
