A quick glance at this - What Is Commotio Cordis? - suggests that the blow needs to be more concentrated as from a baseball, cricket ball or hockey puck and it needs to be in a particular place. Fists are quite large and comparitively soft by comparison.
A fist is more likely to cause it than a shoulder tackle though
A punch from a trained 225 lb fighter delivers far more energy than a baseball moving at 80 mph.
And far more energy than a stiletto moving at 1 mph - but the stiletto could still kill you if applied to the correct place. Maybe more than energy needs to be considered?
A pierce from a knife is far different than blunt force trauma. I fought MMA for almost 10 years. I can tell you getting punched by a big dude hurts far more than getting hit by a golf ball moving at 100 mph and I've been hit by both.
Maybe it is not connected to pain levels?
Reading the literature, timing and placement are key.
And how big is someone's head that's wearing a helmet?
