Commercial flights worked just fine since the 1950s, and now all of a sudden this happens? I call BS on this explanation.

As someone who has worked in government IT for nearly 20 years, it's more likely that a government procedure introduced damage to the data file rather than not following the procedure.
The reason we don't always follow procedures is because they're written by bureaucrats who have never opened a command line in their lives, but have been put in charge of making the rules for things they don't understand.
