I have read of commanders being removed beginning last week. It’s always good to remove a commander you don’t trust or have faith in before engaging in battle. I will have more faith if I see Miley removed as well as the woke members of the Joint Chiefs.
Out of curiosity, who actually decides this? Is it from complaints of soldiers against their commanding officers and a subsequent investigation? Is it started from above (for example, due to insubordination)?
It can be either. Complaints of mishandling reports, mistreatment, etc are directed by Seniors to be investigated and can cause it. Like the Officers that got fired for not getting shots it could be insubordination

Chapter 18 of the Law of War Manual issued June 12, 2015, renewed Dec 2016, has relieved over 120 Commanders since June 2017 for loss of leadership.
Military Occupancy (also in Manual) is cleaning out those who defy the Laws and Orders and replacing with new leadership.
