Like BatteryBaron said, this was a training exercise. I suppose you are accustomed to going to extremely hostile environments and not practice emergency procedures? Especially when you can die in minutes, if not seconds?
High end athletes, special forces, etc all know about S.A.I.D. = Specific Adaptations to Imposed Demands.
The real world unpredictable demands will include oppositional, environmental, and mental/psychological. You can become proficient in all the mechanics of training, but you still can't successfully execute your training in the field if you haven't prepared for specific scenarios where things go wrong, especially for example unexpected stressors or variables come into play during whatever task you've trained for.
