Keep in mind that RFK Jr. is primarily dropping out because the media will not give him the time of day in terms of even negative coverage. Also, he is being forced out because his party didn't allow him to mount a primary challenge, which would have been most likely successful had it taken place.
This appears to be for Arizona only. Although I don't know why it was notarized in Los Angeles, rather than an Arizona county.
I think he may have to do a separate withdrawal form for each state that he was to appear on ballot.
I believe he will need to Officially withdraw in each state..
It doesn't matter where it was notorized - just that it was. If he was in LA he would just go to a lawyer's office, a bank, or even whichever hotel he was in if they have one on staff, get his doc notorized, fax or email a copy immediately, and mail or hand-deliver the original with the raised seal.
I've had to do quite a few of these notarized docs for my mom in hospice in FL, while I'm in TX. Very common.

This document can't be fully legal. As long as a write-in block is on a ballot, anyone is eligible to have their vote counted if their name is written properly on the ballot, and the person is constitutionally eligible for the office. Otherwise the state is disenfranchising that voter. Number (2) on the document can't be enforced. That doesn't mean the ex-candidate can be forced to serve of course, but if someone takes the time to vote it should absolutely count.
Note - constitutionally eligible rules out counting votes for Mickey Mouse, but it doesn't rule out someone who withdrew. Nimrata will still get some votes in this election I'm sure.
