Using the actual redact tool, you would select the text you want redacted (and Acrobat would draw the box itself) and then hit "Apply Redactions". That would effectively replace the underlying text with the black box.
I do this as part of my job and it would be hard to make this "mistake". The tool is literally called "Redact".
Asmongold clips guy reckoned it was because DOGE (+Musk) removed all the Adobe Pro accounts, so the redactions were just a black highlight, not flattened.
