The one time wifey took the dogs for their first visit to a new vet, they got her for $2k. Even though they came in with records the practice treated them like they had never been to the vet before. I took them the 2nd time, and had the talk, and they were much better. I think the annual for 2 dogs was like $500. And when one needed surgery on it's knee, the vet did me a huge solid on the price and follow up care.
If you don't want to be taken for an expensive, emotional ride, just let the vet know up front that you're not one of those "do anything at any cost" people. They usually get it. If they're so well established that they're too busy at any price, that's probably not the best vet to use....look for another that is more in line with what level of care you are able to provide.
Your response is exactly why one cannot recommend any particular pet insurance plan. What you want, is not the same as someone else. Also, if one has a livestock, farm, or ranch background, the idea of minor vet care being done in office by a DVM is silly. No, I'm not going to pay you to clip nails or clean ears. I'm not going to drive them to a clinic for that.
But YMMV. I've had folks horrified about an IV pole to do fluids on an animal. I've got to tell you no amount of explaining stops side eye regarding that.
I am their vets most of the time. Only if I truly need to have some kind of imaging then I need help. Otherwise, I use natural meds and homeopathic for them. The only reason for them to go is if I cannot figure out what's wrong with them.
