The VA issues a card stock, 2-3/4" x 4" "Record of Immunization" with a glued-on holographic seal over the signature. It is suitable for laminating and display. Strange thing is that when I got my second shot, an extra one, not filled out but with holographic seal already on it was on the nurse's desk. I was going to cry out, "LOOK! There's Elvis" and grab it but she was a land-whale and her arm-fat-wattles covered it just as I was going to go for it. So velly solly!
I usually carry mine with me daily. I have 2 sizes and they came with neat little carry cases that clips right onto your belt. I never want to have to use them tbh.
I have my shot record from the Navy. Some of the diseases that they vaccinated me for are horrific - plague, yellow fever, more. I got tuberculosis in the Philippines (could have been worse lol). I will NOT be getting a Chinese bat virus shot.
I imagine it would be like the type of paper business cards are on. Matte, textured maybe, no shine or smoothness like copy paper. Cardstock or thicker. Some kind of pale yellow or blue color.
I consider myself an art admirer and would love to buy a piece of your art, if you are so lucky to get one model for you!
I'm also an art connoisseur now that my firearm collection has been lost in a tragic boating accident. Can't lose art on a boat, amirite?!

My cousin got Moderna vax, glanced at her card when picking her up, don't recall seeing it shine so I think skip the lamination
Some docs are invalidated by lamination, like Social Security card, my dad found that out the hard way lol

Size is exactly 3”x4”, seems to be about business card thickness, maybe slightly thinner. Definitely thicker than construction paper.

It's a bit thicker than paper. I don't have one but my coworkers do. Check ebay. Lots of blank cards being sold.

I know that everyone has see those internet Dr's that will give you a Medical Marijuana card, so I bet you can find the same thing for a COVID ID.
