Depends on where you are, I guess. MIL worked for major hospital in my area. If she refused flu shot she had to wear a mask for flu season. No termination.
Same for us. If we refuse the flu vac we have to wear a mask. My husband claims that his work makes it almost impossible for them to decline but I think he just doesn’t care and doesn’t want to listen to their crap. If he really wanted to he could refuse it, he would just have to wear a mask and they might try to pressure him into it but he could ultimately refuse if he wanted to.
They tried slipping it into our labour laws here in Ontario (Canada), under the COVID emergency measures. They've since retracted it. I have no doubt they'll try again, if things continue on their current trend.

There's at least one University here in Canada which has told staff/students that they won't be allowed to return next Fall (unless vaccinated), provided in-person learning re-opens at that time. That's effectively forcing the staff.
Unless you’re actually working at an old age home, I see no reason no justification for a company forcing an experimental medical treatment on an employee. Sue their asses.
Even then, there is no reason to take an experimental vaccin that is not fda approved, especially when there are viable medications available.
The lack of accessibility and the constant barrage of propaganda may make it seem the vaccin is the only viable option. It is not.
There is ample reason to counter a push by an employer to force you into vaccination.
-1. it is not a vaccin, it is a gmo experimental medical procedure
-2. that requires your freely given consent, so no pressure is allowed. Meaning, an employer coercing you to take it, is in breach of international treaties (law of the land) and potentially is criminally liable in the blackmail department.

-5. Since testing is unreliable, and actually a hoax, as it does not proof you are sarscov free, forcing you to undergo an invasive medical procedure, would add to points 1 -4.

Doing what you are proposing is opening you up to potentially dire consequences, and no amount of reimbursed damages can cure that. This brings us to the issue of relief. FRN's may look like relief, but in nominal and intrinsic value, it is not. So, maybe, slipping in a payment in gold would an additional scare device.
I would look at your proposal as a strategic device, to throw a spanner in their efforts. No more.
This is teetering the line between discrimination and liability... If all comes to down to you having to sign an agreement, read the fine print and ask questions! And if it stinks like motives (use you’re intuition) seek out a trusted legal source
You can ask them to sign, but they have no obligation to sign. Most jobs are "at will" employment. Companies do not need a reason to fire you, and they can only be sued for provable discrimination against a protected class (age, gender, race, religion...).

In the idiot state of Illinois - it's mandatory to get the flu vaccine if you are a healthcare worker - for ALL - even non-clinical workers!

I am as well; my employer just made the vaccine mandatory this week. I have to receive it by September 1st...looking for a way out!
