My neighbor and friend got J&J and it screwed him up for life. At first it was just blood clots but now it is renal failure. He’s recently had a port placed in his chest for kidney dialysis which he will require for the rest of his life.

Please look up life expectancies for patients with renal failure requiring hemodialysis.
Don't let your friend know of what you find, of course. Just advise him, like any adult should be doing anyway, to get one's financial affairs in order, etc.
The renal failure was probably the hospital treatment. They are using remdisivir which shuts down kidneys and other organs and builds fluid in the lungs. It is one of the ways the hospital ensures death.
Technically, the NIH defines the protocol. The hospital just follows it and gets immunity from the CARES Act:
