You might also remind them that's THERE IS NO FDA APPROVED VACCINE AVAILABLE IN THE UNITED STATES, contrary to what the Liar in Chief said. Comirnaty not available here till 2023.
Technically its not unless you work for a medical provider. HIPPA only applies to them.
Most companies are allowing a "elect not to disclose" that will be treated as unvaccinated response.
Either way, it's better to get fired over refusing the vaccine instead of refusing to disclose. You have a way better court case (especially if you push religious/medical discrimination) over the vaccine mandate versus the "disclosure" mandate.
