There it is. Hallmarks of mental problems. Weapon was improperly secured. Keys should never have been in a place where the child could access them. Especially if behaviors indicative of psychiatric illness had been observed.
Tragic. Truly. But once again it seems the only people at fault are the family who failed to do anything to address an issue or take reasonable precautions against any incidents.

I would add that when law enforcement or social work agencies respond respond to such complaints, they should say in writing, "If you have weapons in the house, we urge you to secure them so that your child can never access them, as he/she is showing symptoms of behavior that are consistent with other young children who pull triggers on weapons."
