I remember Rush would say it was something to the effect of them believing they are USA royalty. In their mind they are baffled that anyone would even question their abilities. If true these people are truly sick, but I have always thought Rush might be onto something.
Personally I view it as they are potentially possessed.
What else would explain outbursts as this? Even their sheep become entrenched in their minds that their public servants are their kings and queens.
