I'm working on turning this into a .pdf. I noticed the last section has a "Notary As Jurat Certificate" area. Does this have to be notarized? That would make sense since you're calling for an audit as a Michigan citizen.
Also, Josh Barnett could have helped us out here and uploaded a .pdf. I'm sure one exists somewhere but wtf?
