Fun fact - most people with Q clearances actually got the DoD Top Secret first because the system they use (EQIP) is easier than what DoE tried to use. Once they have their TS the DoE is able to basically pencil-whip it over to a Q. They are the exact same level. The only difference is - nobody gets a Q first so they can get a TS and work for DoD.
So there are tons of people with a Q and a TS in DoE, but there are way fewer with a TS and a Q in DoD. Where people get confused and try to say Q is the highest security clearance is this point exactly. With a Q (and need to know) you can have access to nuclear secrets, and normal DoD Top Secret by virtue of the TS you also have. Keep in mind though - all it takes is the DoE to pencil-whip your TS (and the need to know) and you have the same access. It's all administrative paperwork. Just like SCI access. You can have SCI access for your job, but at some point when your job changes they can revoke your SCI access. You still have a TS, but you no longer require the SCI. If your job changes again the security office calls you in, briefs you again, and you have it again. Paperwork.
Also all classified is highly compartmentalized. Everyone requires the clearance, the need-to-know, and authorization to see any classified material. Some Special Access Programs might only have 5-10 people in the world that are authorized to see the info. Someone else might have the clearance and possibly even the need to know something from that program, but if the point person for that SAP decides they can't read anyone else in that other person will not get to see it. Note - Hillary should've gotten at least life in prison for having SAP on her bathroom server on the internet...
Sauce - I had a TS w/SCI access for 10 years in the military, and a Q for 5 years as an NRC cyber inspector. My TS was expired when I went to DoE years after I got out, so the NRC put me through the DoD system using EQIP (way better than manually filling out an SF/86) and once I had my TS the DoE gave me the Q.

Disappeared for TS? I have yet to see someone convicted of leaking TS other than that weird chick Reality Winner and she's already out of prison.
