Me too, I’ve taken ivermectin numerous times, paste and pill, and never have had a bad reaction from it thank goodness, and neither has any of my family members. We took it a few years ago when the whole family got sick and it helped all of us. The last few weeks I’ve been taking 12 mg once a week to see if it will help with blood pressure and acid, as I’ve read here on the forum that it has helped others with these things.
And as you said, when taking it when not sick, makes you wonder if it is helping or were you clean to begin with. I wonder the same thing. Though I think it is helping in some way, because we feel no different means we may be clean to begin with as far as parasites are concerned.
I think it's how far gone your auto immune issues are, and ability to flush toxins that are produced by the die off. I ended up in a very bad herx when I was ttrying to kill off the lyme parasite babesia,, because I've had the Lyme so long, my mast cells have been building up over the years, when I did a cleanse and used a bio film remover, all it took was a few high histamine foods eaten after two weeks of the cleanse, my immune system went bonkers and I went into anaphylaxsis. Vomiting pain completely covered in hives, there was no skin, just hives.. The mistake was I should have eaten very very simple mono diet, anti inflammation , etc, while I was doing it. And drank tons of water, which I did not, I am really bad about drinking water.
You might have been clear. Unfortunately, in the past I always had a HUGE appetite and have eaten lots of strange things from around the world while traveling for work. I didn't have an obese BMI previously but I'm now a "healthy" weight for my age per the charts.
I have a large appetite due to high metabolism, clinically tested to be about 2900cal/day. It made going on a cut easy but putting on new muscle is a pain right now as I was still trending down weekly even on 3200 a day.
I never experienced the herxhemier when I tried it which made me wonder if it was even working or if I was already clean to begin with.

IVM certainly changed my system for the better.
