There is no top to the pyramid, no high table, not even satan. The current satan is the nominal head but we can't necessarily even know if he's the original or if one ever replaced another, because you could only become the new satan by taking over the identity of the old satan. Note that based on Quigley's exposition of Rhodes's will, the goal is for each "inner circle" to think they're the "innermost" circle, but the reality is that they're each unique and have separate turf that keeps them "happy". The participants of every coven want to become the final antichrist, they kill and steal but can't have what they want, because that seat is reserved only for the one who does it most spectacularly and finally (and who has the biggest fall coming). Because each circle has some unique power, they entitle themselves to think that they are each the most important and "control" the others by their own power being regarded as the most dramatic. When there's a real challenge between two, which is rare because they know the cost, they fight to see which power really is the most dramatic. Among themselves, rather than "open war" (which is too honest), they prefer to make ever-more-secret circles of alliances, and if the new circle votes a former circle member off the island it's just too bad, a day's work. The "survivors" get to rejoice at their "superior" machinations, while the Victim gets to be remembered as the virgin in the volcano, and go to his/her reward, whatever will be the reality of what he thinks the demons promised him.
That's the structure, and none of it need be hidden from view at any phase. People think it's hidden because (a) they do use shell corps and (b) they pretend there is more hidden than the reality (just as they pretend to be older and bigger than they are). But the reality is that nothing is hidden from God's anointeds and all will be revealed at the right time; shell corps are tracked by ordinary human means, and pretense is tracked by both ordinary and extraordinary. They've never even successfully built a pyramid, all they have is stolen from reality and used in the attempted manufacture of irreality, so we have no fear of their claims of being on top as they are proof they're not.
