I also have a gut feeling on this. There is something uncanny valley about her beyond just "ugly woman" and "dude" is the only thing that fits the ticket. Would explain her extreme psychological disasters too.
There are plenty of ugly women out there, plenty of women who have masculine features, and plenty of ugly women who got plastic surgery to (hopefully) improve their appearance (not condoning or criticizing that).

From what i can see, Looma was pretty darn ugly pre-surgery. if she looks fake and kinda ugly now, I'd say that's an easy explanation.
