the OMB expiration basically means that if it's after that date then the agencies collecting and storing the data don't need to respond when doing so. (to save on paperwork)
It's part of the Paperwork Reduction Act.

That date is completely unrelated to the actual document.
That's just the expiration for the OMB approval, it explains what this is for at the bottom in small text.
