Send an email describing the problem with your username to [email protected]. I had the same thing happen after Spectrum's network went bonkers in late July and a lot of pages stopped working. After a week, Spectrum figured out the issue, but Truth never would let me log in again until I got with their support at the above email.
They can look up the lot number and the place it was administered to contact and verify that you got the jab on that day at that place.
Whether or not they actually do that is a crap shoot.
I think there's companies that will do this verification for businesses.
