I think they replaced the original. I don't see how a person's ear can change like that. The new Biden also has a different signature, there was a thread about that earlier today.
Find a better side by side and focus on the skin around the bridge of his nose in both pictures. You’ll see that remains the same. Its still him, he just went too far with the plastic surgery

I have no doubt the current Biden is an actor. Pretty good one, many are fooled even though the faces are not the same. The bigger the lie, the harder it is to convince someone they have been deceived.
The earlobes alone can be explained with plastic surgery.
What is better to consider is the mannerisms of the old Biden vs. the new Biden. If you focus on those things, you will notice that this "new" Biden is very different than the "old" Biden.
You will notice one of the "creepy" mannerisms of the old Biden has not been witnessed even once, that I am aware of, with the "new" Biden. That kind of sick habit doesn't go away. Especially with someone in their seventies when it tends to be an ingrained habit by then.
Also, go back and watch some videos of when he was speaking when VP and look at them now. This "new" Biden uses his hands in a different manner when putting emphasis on certain things that the "old" Biden did.
I am not saying this "new" Biden is fake but I am noticing some things about him that just seem out of character for the "old" Biden.

Noses keep growing through life as they are cartilege more than bone. Also they droop with age.
Biden looks like he had a nose job.
His nose is smaller and not as crooked...it doesn't look droopy or like it's grown, but it looks shorter.
He could have done that. Not much else he could do to look younger.

It can't! I think someone is signing rather than him or something which we can't explain, but his Joe Biden signature isn't this current one.

Working on a similar set, it is NOT the same guy from 2017, and hasnt been since at least 2017.
