You could also ask for the proof of a isolated strain of the Sars cov2 virus and the variants that they are vaccinating you for.
That would be considered "unreasonable cost or burden on the company" where I work. These questions put the company in a legal loophole. Plus they'll never find a Dr to sign that. This is pretty good!

The freedom not to vaccinate shouldn't require religious belief. One should also be free not to vaccinate based on one's logical assessment of the facts and one's right to one's own body.

Correct. Send this letter with a 30 day review. This is the beginning of a civil procedure. During g this time they can not take any actions toward you. If they fail to respond you win by default and can take them to federal court. Don't use state courts for this. You can also rule in your own favor. Anytime anyone got wants to fuck with you, 0lace them in checkmate with a civil suit. We need to do this same as the xNadian guy who demanded a sample.
Might not hurt to get a signed and dated statement that they have received your requests. That will give them the notice that they are entering into a potential legal showdown.
If they refuse to sign acknowledging your request, document their refusal, record it if possible.
Generally, it's a good strategy to NOT refuse, while ALSO putting the burden of proof on them.
In addition, the drug makers themselves say that their drugs have NOT been tested as to effectiveness in preventing the spread of Covid. Let that sink it: these experimental drugs HAVE NOT BEEN TESTED AT ALL FOR THE SPREAD OF COVID. They have ONLY been tested as to reduction in symptoms if someone "gets Covid" (whatever that means).
Since preventing the spread of Covid is the ONLY purpose of requiring a vaccine, they have no legal basis for their demand when they claim they are requiring it to protect others. Taking the drug does NOT protect others. The drug makers themselves say so.
Make them show PROOF that taking the drug will protect others. They can't because such proof does not exist (and probably never will).
Finish up with a statement that, if you do not take the drug, then you understand that, according to the Pfizer study, you will have a 0.71% increased probability of getting a mild symptom, such as a headache, and that if you do get sick, there is no increased risk of spreading it to others, since the risk of spreading it has never been tested by these drug makers.
Most of the people demanding the vaxx are doing it because they are told to, because they are lied to, and/or because they are making money by doing it. NONE of them have read ANY of the studies, and have NO IDEA what those studies do or do not say.
By forcing them to learn (for the first time) that the studies were NOT about spreading Covid, their justification (in their mind) will be gone, and they will not know what to do. It also sets the legal position you need to recover monetary damages if they pursue any action against you, because they will be committing fraud, which carries triple damages (and might also be a criminal act, in some cases).
Another thing to potentially add is since it's a requirement for employment, how can I remove the vaccine at the end of the work day or at the end of employment.
I like this. Combine this with everything else. Make sure they know it.

I like the idea of these things, but I tried the one from Frontline docs for my daughter's college, and they simply ignored it with a one line response...

verbatim, word for word, no opening, no closing, just that line. They just went around what I sent, Granted I sent it anonymously through a new proton mail account I created just for this purpose, citing I didn't want to put my child's name on any sort of list to be segregated, watched, or ostracized if they choose not to get the jab (which they never will), so maybe their response was in response to that approach.
But they neither provided answers to those questions nor did they acknowledge anything risky about the vaccine. Useless.
