Spouse will be selecting option 3. Not too sure what type of company survey wants your name except for a one that's looking for trouble.

My wife’s work did the same thing today, the result was 75% of his management and 40% of his employees would quit is required to take the jab. So he isn’t going push it. Thank God

Employers can legally ask but employees are not required to answer. That does not, however, stop employers from "releasing" employees who refuse to answer.
Sounds to me they're looking to see if it's safe for a mandate or not. If there's too many people who didn't vote one, methinks they may back off.
My company did that too. I proudly told them no and I will never be getting it either. I don't give a shit if it costs me my job or not...as long as I have my health, I can get another job. I think they were disappointed to see that 50% had the shot while 30% responded no and the other 20% elected not to participate. So those not participating were definitely no votes that opted not to reveal their status. Ever since then if anyone from HR asks about the shot, I simply ask them back when their last pap smear was and then things get akwardly silent. Then I politely say, "oh I'm sorry, I thought we were discussing personal health matters." Gets them ever single time.
Testing the waters...... to see how many would quit if they make it mandatory.
This is the same company that when President Trump wanted to tour their brand new "green" facility, they told him no for fear of losing business. As soon as the "sons" took over, the company has been more left leaning. It's quite sickening.
Yup it's a ADA violation as well. Decline to answer, do not lie and say one way or another. If he is vaxxed down the road we may treat vaxxed like the biohazards they are.
My husband received a similar email from his employer a few weeks ago. His small amazing company was bought out by a much larger company, so over the past year and a half they've been implementing their culture, so much time of his workload is attending woke meetings that have nothing to do with electrical engineering instead of producing. His division only has 25 employees, some vaxxed other's not but the majority has ignored this intrusive email and refuse to answer. I will pray for your family but please resist the pressure.

My company sent an email that was to be anonymous , asking what office you were in, if you were vaccinated. Nice try, I didn't reply.

Not ADP. So badly want to post on 3rdReichbook to see what other coworkers are saying but spouse will definitely blow a gasket and I don't want to cause any trouble. If they take this any further, I will get an attorney involved. Am considering an anonymous letter to said company to see if it has any impact.
