Who has custody of the children? If she has custody, there's not much the ex-husband can do. If it comes down to it, she should move with the kids and keep her new location secret. There's nothing more important than protecting your children from harm.
They have 50/50 custody because he’s a manipulative SOB, wayyyy too close to his mom, and also a raging narcissist. He openly flouts her wishes and pulls the kids from school/takes them out of state without permission and STILL has 50%. He is not above getting them vaxxed to mess with her head. This is a bad situation.

I would make sure by court order if necessary that the father would accept all liability for any injuries, side effects, etc. They usually run scared if it means they have to pay.
The courts have been taking children away from anti-vaxxers and giving full custody to the other parents in many cases.
Getting the courts involved in this decision is the last thing you want to do.
