If people were smothered by the crowd, it seems possible that the cardiac arrest was a result of people being in a position where they were physically unable to breathe.
If you've got blood clots oozing around like cranberry jelly in your arteries, a sudden-onset, extended heart-pounding situation like a stage crush might dislodge and push those clot-snakes up into critical systems.

I've been in crowds like that my whole life. Down in the front getting squished till it hurt. No way was that the cause of this. No way in hell.
