This is over the top breach of privacy as a required condition to return to school. Something that we all pay for with our taxes. Additionally, after reading this I will be extra cautious because it is entirely possible that some school districts will attempt to slip in "consent to vaccinate" clauses.
They're doing this literally because they want to take your kids if you're conservative. If they find out you own guns and ammo they will call DCS and your child will be kidnapped under the color of law.
The threshold for a child protection action in most jurisdictions is "suspected" abuse and neglect which includes "possible danger" to the child. No hard proof or evidence is required. The information gathered through this consent form will be sufficient to raise suspicion towards many households thereby enabling a protection action. Suspicion is largely subjective. The authorities are telegraphing their move.
That's kind of why our criminal justice system was set up the way it is. It was set up under the principle that it's better to let a hundred guilty men go free than it is to jail one innocent man. That's why If you get a good lawyer, most of the time all your charges are going to get dropped. Civil court is a modern invention that was created to get around this legal threshold so that the government could oppress the living fuck out of people without really having to prove anything. I know it's fucked up to say this but it's better for society overall if the government doesn't have the power to remove a child from an abusive household except in the case of an arrest and conviction. Because if you give the government this power, then this shit is what's going to happen. That power will be used to persecute dissidents.
Homeschool NOW, or put them in private school if you can, even if you have to reorganize your financial situation.
If they are putting this in the paperwork, just imagine the indoctrination that will take place.
I homeschooled… We took a huge pay cut, and moved into a smaller home, but it was one of the best things I have ever made!!

I didn’t trust what the school was doing then, because of the changes taking place. They took phonics out of school, and introduced sight reading. They also stopped teaching cursive.
They wanted to label my son dyslexic, and put him in special education classes, They said he was slow at reading and math. I immediately took him out and homeschooled both of out boys with Abeka Christian Homeschool Program. Oh, and taught them both cursive.
Now our oldest goes to Texas A&M, and is a Physics major. If I would have allowed them to “label” him, who knows what would have happened? We have to stand up for our kids. We get one chance to do it right.
I feel for everyone with young kids in school. I know it’s got to be hard.

What parent in their right mind sign anything like this? This is very wrong. They should be asking, "does your child know their ABCs, count to 100, etc, any allergies would be one to inform teachers, but other than that, bull crock.
This is bad. Any kind of screening is just a collection net. First was TeenScreen, I. 2007 or so. It was a net to capture kids and give them psych drugs. It produced 84% false positives, and anyone sent for further screening to a psychiatrist ended up on meds. Maybe this is a net to get the kids away from those crazy Christians. Watch out and do not allow this!!!
Biometric data... i.e., the password to your phone and laptop.
The school, and who knows what third party will forever have the "right" to unlock any device secured by a fingerprint or iris, not to mention uploading DNA into a global database to further track and possibly incriminate later in life.
I have two iris and ten fingerprints, that's it, when they are stolen or lost because some "third party" did not encrypt the information and it was hacked, what is the alternative? Not to mention the whole search warrant angle.
