For the love of God people, stop falling for every little thing that people invent. One of those photos is a super high megapixel, high resolution pic and the other is a 240p super low res capture from a news channel. Of course they're gonna look different. It's inevitable.
Here's a few indications they are the same person, just one is more sweaty and has a tan (right).
In the throat there is a dip in the wrinkle leaning toward his right side. From it, you can see a trail leading up and to his left where it undercuts his throat grizzle and also to the bottom to his left and tucks into the collar. These wrinkles would be nigh impossible to replicate in surgery.
The ears check out too. Same pitch and angle.
I can't comment on facial features, because he is giving a different expression in each. The jaw is in a different location, as well, so it makes using measurements of the head difficult.
Eyes are the same color, and have the same starburst pattern.
The hair is longer on the right, but the graying pattern is the same.
Density of his left eyebrow lines up too, with a thin strip being notably darker and having a double-hump like a sketched seagull.
Finally, the fold of his left eye has a crease extending into the radix.
My conclusion? Same Pompeo with a tan or a really good deepfake that also does necks.
The loose skin underneath is the exact same shape in both pictures too.
It is Pompeo, but just a bad photo. Yes, he lost weight and grew more hair.
Look at the eye color. Image on Left has good color rendering, so eyes look hazel-blue. Image on Right has poor color rendering, possibly through a filtered lens or just bad lighting, so the eyes look more brown. The same lens would make his skin appear at least 4 shades darker than it is. Skin tone is likely a combination of getting a tan + bad photo coloring.

Look how tight the colloar is. That is why it looks like that. Added with the weight loss, the skin is looser and pinched.
