No, the mask indicates that you do not have faith in God, faith that He will protect you, or alternately, that He will decide it is time for your life to end. If God is in charge, I do not have to take special precautions like a mask to survive. He is in charge and I follow His will. That would be the best religious objection to a mask. If I die, I die, and that is His will. No mask for me!!

The United Staes of America is the greatest nation on earth founded on Judeo-Christian Principles. One of those principles is that we are created in the image and likeness of God. That image is seen the most in our face. That is the image of God. Your mandate to cover my face is in direct violation of my sincerely held religious beliefs.
You can literally start a religion based around anything and get tax exemptions, etc... Start a new sect of christianity and call it "Constitutional Christian". You worship God and the Constitution... (basically patriotic American)... The mask is a violation of my god given rights under the constitution, and is therefore against my religion... Honestly... As a matter of fact, people who think we can wear them for prolonged periods and be completely fine are fucking retarded. Its probably easier to get a medical Exemption. I dont think they can ask why for medical Exemption. There are plenty of good doctors that can give you this exemption.

Keep in mind that the First Amendment says, "Congress shall make no law respecting an establishment of religion, nor prohibiting the free exercise thereof." The federal government cannot tell anyone what is or is not a religion, nor what is or is not a religious belief. No authority at all.
States have the same prohibition via the 14th Amendment and the state constitutions.
Private companies are following a federal government agency's (CDC's) "guideline" which is NOT a law, and are doing so fraudulently, since the guideline has no scientific basis behind it.
I might consider something like:
"My religious beliefs are my own, and nobody can claim otherwise. I believe strongly that nobody may bear false witness (i.e. lie), and I believe I am obligated to expose those who do. It is a violation of my religious belief to falsely claim that I must wear a face mask for a purpose that has no truth behind it (i.e. it is a lie), and that I have an obligation to expose those who do promote such lies. I believe in the Book of Genesis which says that God created the Earth and the air perfectly, as is."
If they do not accept your "claim," you then at least have a factual basis for a lawsuit against them, because they are basing their policy on fraud (no scientific proof that wearing a face mask helps anyone, and plenty of evidence it harms the wearer).
As far as not having enough room, if it is paper, write "See attached" and put it on a separate page. If it is electronic, state "This electronic format does not allow room to express my religious beliefs, so I will need an alternate method to do so."

I think that an employer, especially a non believer, might throw back in your face, that you may be made in the image of god but you still wear clothes and don't walk around naked. So I don't think that will work with them. It's going to have to be something harder to argue back. I can't find any scripture talking about not wearing face coverings, but I did find this article that might give you a few extra ideas. I agree with the writer that it is dehumanizing, and you can say that is why we Christians don't mandate our women cover their faces. You tell them it is YOUR religious belief, not theirs. This is not a subject for debate. It is a subject to state a belief. They have no ability to prove you don't believe it.
Maybe so, I know people are getting bullied pretty bad so I guess it depends how much of a fight you want to deal with.
Legally, it absolutely is subject to debate, since the religious practice conflicts with what the company and government states is a safety standard. You can claim a religious objection, but an employer absolutely can ask you to prove it and call bullshit if they don’t believe you.
The analogy I’ve used before: pretend you want to start a bonfire in your cubicle and justify it under religious freedom. You’re free to make the argument and might even be truthful, but since starting fires in the office is considered dangerous to everyone else in the building, you’re going to have a tough fight.
As far as many companies and institutions are concerned, it’s the same argument.
