IF he had a lot of plastic surgery to pull his skin back, that could pull his earlobe down and have seen shots from behind his ear where it does look like a surgical scar, I was watching an old Joe clip and his face was more fleshy, eyes more hooded but, certain expressions even then, show the same eyes of now (when the skin is tightened). Just my personal view off what I could see over multiple clips/pics.

And I think a good doctor would have kept his detached earlobes for integrity (lol) but clearly this doctor wasn’t the best. But other details that could be off are there. Looks like he also should give up the tanning bed for a while.
