I absolutely did this and REALLY gave my Dr a hard time regarding the masks and injections. He's probably 20 years younger than me, wears an N95, a regular Chinese mask and a face shield and conducts 90% of my physical exam over the phone. He's completely convinced people are dying all over the place from the scamdemic.
I forgot to point out to him that the health questions on the tablet they give you are provided by Pfizer and the WHO (it states this on the screen as an advertisement). I wish I had time to take a photo of it.

I think the more plausible scenario is that it's a generic web form for appointments used by some web company that's probably way overcharging. I would say that's predatory, but on the other hand, fuck the medical system, they're the personification of that.
I think some of those true hermaphrodites were inspiration for early sex assignment surgeries. Those surgeries might’ve been well intended, but of course those who aim to depopulate us would take that ball and run with it to the twilight zone

It's crazy to think that we're allowing this even in medical environments. The health of trans people is directly affected by their birth gender. It doesn't matter what they believe they are, a woman cannot get prostate cancer, and a man cannot get ovarian cancer, not to mention numerous other ailments that can only occur in one sex.
By playing into this bullshit, you're directly putting them in harm's way. If you claim to be 'inclusive' and care 'so much' (tm) about the plight of transgender people, you would NOT let them pretend in your medical practice. For their health.

Unless your gynecologist is the best in the country, maybe you should recognized that if he/she has this kind of unscientific BS on a patient form, he/she might be compromised and not fully understand anatomy, genetics, chromosomes, and sexual dimorphism. For a gynecologist, those are deal breakers, sort of like taking a tumor to a witch doctor and having herbs burned so that the smoke will drive away the bad spirits.
To be fair, they may have purchased a prebuilt solution to store their medical records that complies with state and federal laws but isn't tailored to their specific practice.
