That is obviously not her voice, and while she does have somewhat severe, masculine looking features, she was not a man. Some women look more masculine (probably like their dads) just as some men are more 'pretty' and probably take after their moms.

While I don’t care for any of the Bush family, one doesn’t need to be transgender to be evil. Barbara looks like many older women who have gone through menopause and no longer have the balance of estrogen and progesterone that makes us women appear more feminine. Many older women of Germanic heritage, who are actually women, do appear less effeminate as they age.
