Yep. The three head rule - men's shoulders are wide enough for a head on either side. Just check out a lot of the closeted trannies ... shoulders, hands, hips - all men. And they are childless/adopt.

You can very, very, very easily and seamlessly put someone's head on another body in photoshop if that is what's being gone at here.
Only takes you a few minutes if you know what you're doing. Getting things to match up so it looks natural, again, does not take long if you've used photoshop for a while. You can find a ton of tutorials online for such things.

Not one picture goes on the cover of Vogue without being airbrushed to enhance the person photographed. Never has and never will. So yes it's photonshopped.
