What helped me the most was progressive relaxation tapes that I would run on a loop while I slept. I engraved that psycho babble deep into my subconscious mind.
Count to ten, tagged with purification of light and warmth, then with detoxification and desire for health, wealth and wisdom...( hells bells two out of three ain't bad, and someday I'm grateful to just hang on to one)...
Get to where you're jonesing the count to ten starts the defusing of the desire / need, then it's easy to postpone the flick of the bic for later, and later, and later yet still...next thing you know your a non smoker.
I just puff occasionally now, but haven't in the last 2 + months trying to get rid of a dry cough, of course I had to temporarily kick coffee to do it, I'm a caffeine fien as well, get the coffee brewing and the pipe billowing, stand aside a fast moving train coming thru.
I can cold turkey stop if ever I want or need to, but I can also puff occasionally and enjoy a bit of adult indulgence, but not have it be a monkey that has to be scratched on demand.

It sounds like you have the smarts to be a therapist, my oldest sister, God rest her soul had a phd in psychology with a minor in education, she feed me a steady diet of self-help books and second hand college books with all her notes.
She had it pretty rough too, and went into psychology to help make sense of it all. It really didn't help her, she was to overwhelmed with guilt and anger and then when she started self medicating she got hostile and isolated. I miss her, super smart but wound tight, she was definitely over stimulated as a child and never learn how to breath.

I just can't imagine going thru life on autopilot, mindlessly playing out some script or game in a hollow, shallow existence, I want to think deeply, live richly and love completely, without hesitation, reservation or speculation.
