An email went out to all employees last week that they hired a third party surveyor to collect vaccine status on everyone. The language basically said that although we haven’t implemented the mandate, they would need to know if we had it along with proof, or if we had not received the jab, supposedly for statistical purposes. We are required to respond no later than 11/22. Everyday since, I have received a reminder that I haven’t responded and that I’m required to by the deadline. I thought that surveys were optional(?). What happens if I don’t?

Tell them when a law is enacted requiring you to provide private info, you will comply with that law. Until then, no.

think about (and prepare) your answer for a case your boss asks you in person, e.g. "I'm not sure I'm ready to reveal the details of my medical record" - something that's not even an answer, but it delays your answer and you don't promis that you give them the answer they need.
Tell them in the interest of personal confidentiality, you will not be disclosing any of your medical history or information.

In a right to work state you can be fired without due process. In other words, you don’t have to be given a reason unless you’re under a contract. I recommend you seek legal council. Those who are citing HIPAA as your protection are not entirely correct. It would be a violation of HIPAA for your employer to circumvent you and go directly to your insurance carrier or healthcare provider to seek medical information about you. If they provide it without your consent then it is a violation of HIPAA. It is not, however a violation of HIPAA to ask you directly for the information. If you refuse, they could fire you under right to work. They don’t have to give a reason. Therefore, I suggest you get legal council on your options regarding answering the survey and what recourse you may have if you don’t answer. Also, document every conversation (date, time, with whom, and what was said.). Print and keep a record of every email sent out regarding this survey.

I don't know if this will help your situation but here's how it got handled in a hospital situation over the last week.
So, I go to the hosptial for chest pains and a blood pressure of 208/112. It happens from time to time.
While there, I was forced to take the "quick" test. Got the results back in 20 minutes. Negative. But 20 minutes, sitting in the waiting area because there's no entering the emergency room without a current test.
So, I get into the emergency room and there's a constant stream of nurses and nurses' aids asking me about my vax status. Ain't had it. Ain't getting it.
No real effort to treat my health issue. Some futzing around. But no effort to bring the bp under control or even reduce it. But I do get the usual horror stories about "all the unvaxed people we have dying up on the ward!" from all of them, constantly, in rotation.
Don't have it. Ain't getting it.
Finally, I call the "doctor" over. Mentioned, calmly: Nuremberg Code. Crimes against humanity. Zero proof the vax does anything beneficial. Overwhelming proof vax is killing and injuring many many people. You are a professional. Your duty is to keep up with info as it develops. First do no harm. No where in your oath is support The Narrative. Everyone in your industry will be held accountable. This is without doubt. Pushing the vax at me is samey same as advocating I submit to assault with a deadly weapon.

Didn't even have to wear a mask anymore.
Got treatment.
4 days later, I'm out of the hospital and doing fine.
This is why I was just fired. I would not respond.

Ignore the survey. Do not respond. Unless there are posted consequences for not responding in which case re-evaluate.
By not responding you are sending a message that your medical record is private and that you are not going to participate.
This non-responsive rate will be reported to the company and will help the exec gauge the level of noncompliance they will have to a vax mandate.

Do not participate: To participate is to tacitly agree with surrendering your privacy and bodily autonomy to the organizations.
To solicit your status is not necessarily a HIPAA violation as so many mindlessly parrot. But it does violate rights protected by the 4th and 5th Amendment. The 4th pertains to privacy, and the 5th pertains to presumption of innocence -- it's up to then to prove that you are a threat; you do not have to incriminate yourself.
The first thing to understand is your bosses CAN, in the current climate, require you to disclose your status and even fire you if you don't.
The second thing to understand is if your bosses are doing surveys like this they are afraid that if they do a mandate they will lose too many people to remain a viable business.
It's time to take a stand and tell people clearly your position on the matter.
I've made my position clear to my bosses before the shot was even available. I've made it clear with them frequently since then. My employer, when they realized they would lose about 40% of their staff, decided a mandate was a bad option.
My employer has federal contracts so we are under pressure to comply.
I’ve made it clear that I’m not getting the jab. This is different.. They want statistical information from the company over all. The part that bothers me is they are requiring everyone to participate in the survey by the deadline.
How about, "I don't disclose private medical information."
3rd Party surveys are none of their business! I would think HIPPA would prevail and this seems to be highly discriminatory! Was your Vaccine status a requirement when you hired on? Were you given an employee handbook when you hired on? Did the Handbook even mention anything about Vaccines or their requirement? I bet not!
