She was the clerk who signed off on it if I remember correctly.
She was on a small plane. It crashed in a textbook water landing with multiple other people. They all went into the water. She was the only casualty despite wearing a life vest.
Yep, that's the one. Everyone survived the crash from what I recall and they were floating in the seemingly calm waters, yet she "drowned". Hazy footage I saw back in the day indicated there may have been someone in scuba gear that physically submerged her
