For a doctor appointment tomorrow, I was requested to update my information and sign a care consent online. The first item of the consent said, "I understand that a patient's care is directed by his/her attending physician(s) and I consent to any hospital services that are appropriate for my care and as ordered by my physician(s)."
This physician seems pretty good, but what if she thinks vaccines, biologicals or gene therapy are appropriate for my care? She did ask me ONCE if I wanted the sars-cov-2 vaccination, pneumococcal vaccine, herpes zoster (shingles) vaccine and tetanus vaccine. She's also offered a mammogram and dxa scan, which I declined. If she had asked again, I would have found another practitioner.
I have serious trust issues with the (lack of) healthcare field. I will not sign this. The provider (hospital system) has sent it to me about 4 times in the last week.
Add an asterisk to that statement, then sign it.
"... and I consent to any hospital services that are appropriate for my care and as ordered by my physician * and has been expressly agreed to in writing by my attorneys. "

It is electronic and there does not appear to be a way to amend it. I do like your suggestion, though.
Ask them to print a copy before you sign it because you'd like to add a few specific instructions, after that's done, have someone there sign under your signature, such as a nurse, and make a copy for you.
I just got one for an appointment this coming week.. just a BP check, that's it. Here's the text... I authorize the Provider(s) or their designee(s), in charge of my or the patient's care to administer ANY treatment including medications(s) or VACCINE(s) as deemed necessary or advisable in the diagnosis and treatment of any conditions related to me or the patient... Amazing.. This basically gives them free reign and I'm on the hook to pay for it.
Just tell them straight up you don't consent to any vaccines when you get there. If they push the issue, walk out and find someone else.
The hospital probably gets big money for each vaccine they administer. Thus when you sign that you are authorizing them to jabb you, on your written behalf, if you come into the hospital emergency as a comatose patient. The doctors will read your consent form, rub their hand together like a mad scientist, and say "yep, let's jabb him/her". You might have told your regular doctor you don't want the jabb, but the attending ER doctor does not know about that conversation. He will see your autograph and say, "let's gooooo".
Maybe the best thing to do is tattoo a resounding, "NO, I do not consent to any vaccines!" on your forehead so emergency room personnel know your wishes. Kek.

Don’t sign anything like that. The system is getting more and more bureaucratic to get medical help. See the doc, and if they insist on this type of paperwork, go elsewhere.

This has been discussed here before, although it’s probably been more than a year since I last saw this. The advice, then and now, is never sign anything on an iPad. Always insist on paper copies.

Specifically, do not sign a Health Monitoring document, at work. Do not confuse this with 'health checks' that are offered as a voluntary 'perk' of the job, which one does not need to sign consent forms for, because to that end, it is just a blood-pressure test with a trainee nurse.
The Globalist trick is to make a list of 'health instruments' back about a decade ago, based on known REALLY dangerous stuff. This was for obvious hazardous things like Asbestos, and other poisonous or radioactive substances. Then, during Covid the shenanigans started: Specifically, Ardern (the lead fish for legal contortions) made it so that they decided that bacterial and viral hazards could also be added to that list. Once one dives in, it is apparent that this is not the case legally, and awake pepes should now be puttin' their foots down. However, councils started monitoring programs for their buidling inspectors - because they go onsite, of course. Very dangerous. /s
Of course, a health instrument must have a specific test associated to it - so for example, one gets regular lung-capacity tests, if it is established that one has been exposed to an unacceptable but measured level. The point is: that the Labour logical dumdums decided that the 'health instrument' was the vaccine. < I actually saw Ardern deliver an impassioned speech about how 'health Instruments' were the way forward to get 'mandates reinforced in workplaces' - Realize it was political goal. So, when (they) proceed to shove health monitoring consent forms at one, realize that it is to get you to be vaxxed, just to keep one's job. Also, if they present those documents, it is a very serious issue because in reality it means either: that one has been poisoned by one's job; or it means a vax-mandate - (when the risk is that one is exposed as is a dirty vax-denier). This is a different but very serious issue, because the whole thing is signing one's life, or job away.
DO NOT SIGN IT. The employers actually have no recourse, because if one is not giving one's consent, they will try threats of unempolyment, and that will be illegal. They will still try, but notice that they won't do that on paper. By not signing it, you are not putting yourself on a list of 'damaged goods', if you like. For later. So, when one has retired, and one has a record of covid vax, LOL. Off with his head. Put any other conspiracy theories here. But needs must.
Be aware that there is a minefield of other human rights transgressions along the way, because health monitoring requires a decades-long program to keep an eye on the victim. So, the employer gets access for 40 years, to one's medical records. < and that' is actually what you are signing.
A good thing to do here is play dumb, like "i don't have a cell phone" & "I don't have a computer" don't ever reveal your hand. I never install apps or give cell phone numbers or anything like that.

Also, when you go into the treatment room to see the doctor, ask if they have software that automatically records your conversation. I just found out 6 weeks ago, our medical establishment, multiple hospitals in multiple states, uses the software that does this. I'm glad I quit using my employer for my healthcare several years ago before they began using this particular software. I don't believe they tell patients they are being recorded either. There was a post here about this happening in California healthcare establishments and they are being sued due to the patients not being informed and giving consent to the recording. It may be in our consent agreement but most everyone just signs. I think they should be required to get a separate consent since this is new technology.

My veterinarian does this. I do believe it is recording the whole time because the laptop is there the whole time.

NEVER electronically sign anything
When entering ER nurse comes in and says ,"this is just giving us permission to treat you". But READ it and make necessary changes. Usually #5 or so on the list is "We assume DNR is in effect,". In other words they do not resuscitate. So why the hell were you taken to the ER,? So when you die they tell family "he didn't make it. Here's your bill".
Same with discharge summary. Nurses don't bother asking and fill in what is completely wrong. Got pissed off when I MADE her correct it. Something as simple as ADL normal (activities of daily living) because I never got out of bed. I am disabled and use assistance: now by her entry my disability insurance would be denied, might even be charged with some kind of insurance fraud. Something minor like next of kin, very important when you can't speak for yourself they listed NONE so they could decide on my care per their plan. When it comes to your end of life decisions they say they want your living will (which they somehow always lose) and you make sure it is recorded that your next of kin and attorney have it, you will have to consult with them beforehand. They will say they pull-the-plug unless they have it and your response should be "BY FEDERAL LAW you are required to keep me alive UNTIL told otherwise by next of kin. Both you, the doctor and the hospital can lose your licenses and be held criminally responsible."
If you must, and there is no other way, signing under, or after the words Under duress Signed or printed name Can be helpful

The real question is why you continue to go to doctors, knowing what you do about all the horrible things they do.
Today, I just wanted a prescription for ear drops refilled. I checked with Medicare and a yearly visit is not required, so if I feel well next year, I'll cancel the appointment.
The doctor did however offer a survey to see if I qualify for a cancer screening blood test. She said half of her patients who took the survey qualified for the test. "If your insurance does not cover the test, the max price would be $250." lol - I declined. No more fear porn, thank you. She also said that if I ever changed my mind about vaccines or a mammogram to let her know. haha, not bloody likely!
I think the consent form that I mentioned initially was a hospital admission consent. I think it is good for a year (unless rescinded). They like to keep one on file. No one at the doctor's office mentioned that I failed to sign it.

These are the same doctors that are part of the medical industry that have caused so many problems. We know they're crooked and shady and can't be trusted.

I think I do understand your meaning. I am a retired nurse. It hurts my soul to learn how I and others were lied to and that true knowledge was hidden when it suited a purpose. I did not go into nursing to hurt others, but to serve and care for them. It was a decision I was told that I made at the age of 3. I believe there are many good and well-meaning people still in the industry. Some/many may still be asleep. It was a not an easy choice to see the doctor today and it caused me a bit of anxiety. I think this doctor is asleep to the truth, but means well and shows respect to my choices to decline vaccines and tests. If those who know never interact with those still asleep, then we have no opportunities to share the truth and potentially help them to see. I appreciate your comments and beliefs, but choose a different approach.
I mean they're not going to give you a vaccine while you're unconscious for an operation. I guess it just means you're agreeing they have the right to drop you if you refuse a vax maybe.

I believe that people were jabbed during surgeries. I haven’t read the latest consent forms. I avoid the doctor like the covid 19.
I remember videos where an ER patient was held down by 3 security men because she refused the jab and they did it anyway.

No. That was an adult woman who went to ER for something not related to COVID. In the early stage of COVID they were literally forcing the jab on anyone who went to ER

These are standard consent forms that have been around forever The provider is covering their liability. Basically it means that if you cone in for treatment and they recommend a reasonable course of action, you can’t just turn around and sue them. If they are willing to hold you down and vax you against your will, I doubt they are concerned with consent forms in the first place. Maybe don’t seek medical care in a place where the staff looks like actors from Deliverance.
