Attached is a website for lacounty.gov where you can print off a copy PDF copy of the Covid vaccine card. :>)
Feel free to make up fake information and dates. When putting down the Product name, you can either use Pfizer-BioNTech or call it BNT162b2.
If you want to put down the Moderna vaccine, enter product name as: mRNA-1273 or ModernaTX mRNA
If you want to put down the Johnson & Johnson vaccine, make sure to only use ONE date (it's one shot) and put the product name down as: JNJ-78436735
Make up a fake Lot #: For the Moderna vaccine, use Lot #: 041L20A For the Phizer vaccine, make up a Lot #: EM_____ (make up a 4 or 5 digit #)

I found the dimensions in Photoshop:
4.167 x 3.304in.
Well it just so happens that 8.5 x 11in paper can print out 6 of these and they fit perfectly, leaving about 1/16" on each side.
(8.5 / 2 = 4.25 & 11 / 3 = 3.66)
Print front & back on cardstock for added authenticity.
Careful to cut straight or just use 6-piece perforated 8.5x11in cardstock.

Well I watched a good video yesterday from a guy that seemed pretty up on things. He said you would need 4 of these type shots each with different nano type stuff in them to turn you into a zombie. I laughed and so did they until he referenced the CDC site and thats the exact terms the CDC uses. Its a example at the CDC but could you pick a different way to explain it other than zombies! Anyway its a 10 minute video and then go google the CDC and Zombies and see what you think. If someone made these, I would think it a good idea to carry several copies, one with your real name when absolutely needed and one with a random name. Provide the random one when at all possible and if something pops up problematic, act completely surprised and pretend they must have mixed it up with someone else's last time you provided it. Of course, I will not be making these because I'm planning to follow proper government protocol and receive the vaccine at my earliest possible convenience. ;-)

I'm hoping someone can get a picture of a filled in one so things like a lot number can be recorded with a real one. People will suspect phony cards will be used and may be told what to look for in correct coding for a particular vax.
My boss has one but I don't think he got the 2nd dose yet. His also had a sticker, possibly with the vaccine info. I'll try to get a pic of it next time I see him. It was definitely heavy stock.
The girl that cuts my hair also said I can copy hers (she was going to go get the jab when I saw her the other day). For the most part, people will believe anything you show them on paper if it looks official.
Some places do stickers, some are hand written. Its not too hard to find the social justice warriors showing their cards. #protected #findyourshot are a couple of hashtags I saw used
