Have they been exposed to anyone with tuberculosis in the last 2 years?
I'd like to come up with more applicable questions related to the medical status / information of the (clients). Can you help?

They're being incredibly disingenous. "Decline to respond" is a valid response, it doesn't even require an explanation.
Unlike an EO, HIPAA is a law, and them asking for private info like that opens them up to prosecution for that and for attempting to give medical advice and force an employee to get a medical procedure.

I agree. They are doing the 'check the box. I had a dentist appointment earlier this week. They sent me the text to confirm the appointment and then the link about CV which wanted me to enter in all my vaccine data. I ignored it. I am NOT entering anything into a database - my company does a lot of work in cyber liability and I have been taught well. (Small offices are targets because of the insurance data.) I was all prepared for a fight and all that happened was they took my temperature and asked how my health was. No vax questions.

Tell them "I am waiting for the FDA approved Cominraty vaccine to become available in the USA, sometime in 2023 or 24. I will not be taking ANY "EUA" Emergency Use Authorization vaccines". Which is ALL of them currently.
I'd start with telling them that her provate health info is her private health info. Not yours or theirs and she will share with her chosen medical provider as she sees fit.

This; I worked as a contractor at a large hospital for years that required Flu shots (Apparently; I never heard about this requirement till year 2 as I was at an offsite location that didn’t deal with patients).
I “had” to get this shot they told me. They would ask did you get it yet, and I’d say ya I’m working on it I’ll go this week over and over this would happen. Then one day they would say “hey you HAVE to get your shot by x day as that is the last day they are doing them”. Great I thought, I was tired of hearing about it.
The first time I was a little worried that they might just fire me but then I just didn’t hear about it again until the next year, and the same charade played out every year.. for 3 years till I left to do other things.
I don’t think they forged anything for me, unsure though.

You do not have to disclose this info to an employer.
When doing medical procedures you will be asked about health history. If you want to disclose it is your choice but you sign a waiver.
Don't forget to ask the Aids Hiv status of employees.

Print a copy of the Health Information Privacy Protection Act of 2013. Federal law, they cannot seek your personal medical information , and they can be sued for even asking.

Your wife can forbid her healthcare providers from sharing her personal health information with her employer, and that is the extend of HIPAA protection in this situation.
You're wife's employer is not prohibited from asking your wife if she's vaccinated. However, the employer would be treading on thin ice if they asked more questions like, "why aren't you vaccinated?" or any other questions which may require your wife to disclose details related to her health.
The legal system are a bunch of grifters too. Even the lawyers on our side. Fake language and without moneybyiu get jack shit. Lawyers are lower than doctors.
