As someone who uses photoshop everyday, this photo seems very odd. usually only get that much grain when its a low quality pic or low light. Both of which call into question how detailed her face is. My guess is at the very least the exposure and contrast have been overly adjusted. There shouldn't be that much grain for a face lit with that much light on it.
