It gives a small bio of him and then gives his contact info as an address which is an inexpensive home in GA, not a business or medical office.
No phone number is listed, no website is listed for him and it says, 'where does Dr. Avery practice?' and then it gives this answer: "App Of Indiana Ed, Pllc"
Which says it is located in Indiana, but his address is in GA.
I wonder how many of these are fictitious characters or actors.
