I would have to keep the appointment just to ask them all to attempt to prove more than Two genders, and How it is possible for one gender to Become The Other gender. Cutting off a penis or breasts does not change genders, it simply makes a more insane nit wit.
You and I would be a fun hang-out. I would totally keep the appointment and make a mockery of the whole clown show. As a woman, I'd ask him to please check my prostate, seeing as how I identify as a man. I'd change my gender-fluid mind halfway through the exam and then back so fast they'd get whiplash. I would do it from the moment I signed in until the moment I paid my "money well spent" copay.
My greatest amazement is they all have an 'education' and still 'pretend' that this Insanity of 'trans' is Real and defendable with logic. They are also Insane.
My "educated" millennial is in town, and somehow, we got on the topic of pronouns. I told him I respected anyone's right to be delusional but I didn't have to play along. I'm not butchering the English language by saying They is. He said it takes so little effort to make minor adjustments. I asked him why I was the one supposed to adjust. And what if that person isn't satisfied with my adjustment? What will they demand next? He stormed off upstairs. Hopefully to process.
I'm so glad my 26YO son is so based. I'll tease him all the time about the 48 different pronouns and he just rolls his eyes. Living with a woke individual has got to be a chore. Don't forget, you have control over your estate. IF the kids don't want to be normal, then cut them out of the family inheritence. hehehe

I refuse to argue with insanity and justify their allusions. i may even laugh as i ignore them.
Thankfully, the Greek language has no pronouns, so we are spared this insanity!
As fun as this is, be careful of leaving documented encounters within the medical establishment. They have prepared medical diagnoses and codings that the government in the near future will give themselves authority to view and dictate your life based on how "crazy" they think you are.
Avoid healthcare like the police until absolutely necessary.

Be aware that most of these online digital forms are not managed by the office, but by a third party. Who knows what happens to your data. If you must go to an office, make them give you hard copy forms to fill out that are kept on sight in the office.

Just the fact that they are asking you to answer questions online bothers me. They are asking you to do their work. Find another doctor
I interviewed with a doctor who also had these questions on their intake forms, as well as questions about my sexual behaviors. Even though I replied that I was a straight woman, still, the nurse asked me a slew of invasive questions in the exam room. She asked (with an assuming tone), "Have you had sex in the past 6 months? With both men and women?" I laughed at her.
Later the doctor got extremely angry at me for my stance on vaccines. His face even turned red. It felt like time slowed down as he got up in my face lecturing me. I could practically see his blood pressure rising.

After this visit, I found a good nurse practitioner to use, instead.
I had a gynecologists' office ask what gender I identified as when I was being checked-in by the nurse. I told her I was not answering those type of pandering questions. The way I figured it, if the doctor could not identify that I was female when they did my yearly physical, then they did not need to be a gynecologist. The nurse seemed PO'd, and I never returned after that one visit.
I never fill out these on-line questionnaires; I fill out their forms in the office hard copy. This way I can write in any of my comments.
As per my preferred pronoun, I am waiting to use 'your majesty'.

I have had medical records transferred from one doc to another twice. The question that pisses me off asks if I will allow my records for mental illness/mental health (something like that) to be transferred … I have never seen a doc of any kind for an emotional or mental issue. How do you answer that question? Yes: send those records. No: Don’t send those records. I have written across the question. Send ALL records to Dr. 🤬

These questions don't necessarily mean that doctor believes in gender ideology. This could be there as a screen to catch those who do believe in gender ideology so that they will be prepared to treat them as their correct gender.
Still though, it is depressing and enraging to see this.
Considering how dangerous it can be to treat a person incorrectly (Medically speaking) based around the assumed incorrect gender. This set of questions is likely to cover their ass so that they know if the person is Mentally deranged or not and to be able to apply appropriate treatments as needed... And to know how to not piss off the money source.
