It's up to the discretion of whatever financial institution is processing the check. I'm not going to put much faith in a bank teller enforcing the rules against the IRS.
Don't tie your exemption request to any particular thing that can be proven true or false, like fetal cells in the vax.
Instead, have something along these lines: I'm a Christian, and I believe that the Holy Spirit resides within me. I believe that my prayers are answered by the Holy Spirit. I have prayed about whether or not to get the vaccine, and I feel led by the Holy Spirit to not receive the vaccine.
This is a better strategy, because there's nothing they can do. They can't prove one way or another how you feel or what the Holy Spirit is telling you to do.

You're falling for the false promises. The false promise of the FDA is that only safe food and drugs make it to market, but what we actually see is harmful products make it to market, and lifesaving food and drugs are held back. Similarly, the false promise of the EPA is that they'll protect the environment, but they push hybrid and electric cars which are much worse.
Abolish both FDA and EPA. Push food safety requirements onto the supermarkets and let them do quality control. Also let people buy prescription drugs over the counter like they do in other countries (keep narcotics and other addictive drugs behind the counter).
If you want to protect natural forests, etc. then zone them as parks and let the free market take over with private ownership. Private owners can charge a fee for entry, and they have a financial incentive to keep the place clean and the flora & fauna healthy. (Read about "Tragedy of the Commons" for more info on this.)
Government does a terrible job regulating these things, so it's often better to just let the free market handle them.
I am unsure of how much to dose
There should be weight markings on the tube. Round your weight up to the next notch, and count how many notches that is, and that's your dosage. Ex. If you weigh 220 lbs, round up to 250lbs and see that it is 5 notches. Every dose is 5 notches until the tube is empty.
Biden is the fall guy. He's the bag holder.
The USA needed to get out of Afghanistan, but of course the Taliban would return. It's not our job to ensure the Afghani people have democracy, if they want it they need to work for it. There's no good way to withdraw, so you have Biden do it and take the blame for "fucking it up".
Same with exposing the vax. Many sheep would keep the lockdown running until a vax was available. But the vax is dangerous. So you have Biden do everything he can to make it mandatory, and that makes people do their own research. That makes some judges actually preserve liberty under the Constitution.
I could go on and on, but my point is there's an end result that we want to achieve, but regardless of how we get there, it makes the President look bad. So you have a fake president take the blame. And my hope is that once all of the end state objectives are obtained, President Trump officially returns to the White House.
