The photo is real. AI wouldn’t show his grandfathers ball sack like this photo and the hand is that of his grandfather. Just to be sure I put it into ChatGPT though and this was the response:

You can tell your NOT A PHOTOGRAPHER...there would be a blur there if there was movement of his arm during the shot
I'm not a photographer, but I do know that outside on a sunny day, shutter speeds are necessarily fast (or else you would end up with a completely white photograph), and the old man is hardly Bruce Lee. I mean, how fast does an old man move his hand when it is an inch from where he wishes to gently lay it? he moves slowly, of course. So we have a slow hand, and a fast shutter. I stand by my assessment.
TBH, maybe that is the normal resting spot for him. Who else but Big Mike would wanna touch that?
Seriously though, it may be his palm resting on his shoulder, and his hand is flat so it's actually just flared out, not cupped. Old people do that for any number of reasons.
