If you're dealing with a university and its veterinary school, then it's no surprise. My previous vet before moving wasn't associated with a school, but they also had a mask policy, and once I moved I found another one that didn't care.
There's nothing you can tell these people that will make them care or change their minds. I would just write an email that says until they change their policy to reflect actual scientific data, you and your family and friends will find another clinic for your pets.
I'd call to ask the receptionist if people actually care about it. If they say yes, just cancel. If they say no, just go and make them all look foolish.

How disappointing to realize that they have fallen so far behind both the science and the community that, instead of leading, they have become staunch holdouts of fear and insecurity. With actual scientists proving the case, even politicians and eventually even the CDC have all acknowledged that masks do no good and are likely harmful. Shed the fear and get with the program. Until you do, please cancel my appointment and remove me from all your mailing lists. Sincerely..."
I would point out the date on the notice is June 2021 and let them know even the CDC has admitted masks dont work. If they argue with you, tell them you want more informed people treating your pet and move on with your money and your dog.
I think you should show up to the appointment without a mask on. When you get told to put one on, say you're not the customer, your dog is, you just pay his bills because you love him. The letter says customers have to wear masks and you aren't a customer. If they want Doggo to mask up, they should talk to him.

What a nightmare. Site infections, on had cotton balls left in it, stitches failing. It was a disaster that lasted not even a week. I'm sure in Ohio you have a good, rural vet. Find one of those. And your cancelation letter should read like this. MASKS? FUCK OFF!

Just going to throw out there that leaving a negative review on Google, Facebook, etc. makes a difference and getting the negative reviews removed is very unlikely. It’ll lower their star average for those who don’t read reviews, and it will warn sane people not to waste their time with businesses with crappy practices if they do read reviews.

I told them I cannot wear a mask as I'm being treated for congestive heart failure. I hyperventilate within minutes and will need O2. So call and get a Medic here with the O2 and I'll put the mask on.
Got some hem-hawing and I said: You ARE MY CARDIOLOGISTS, check my records.
Not only did they let it go (I guess more because they didnt want to lose $ if I walked out) but I never had a faster doctor office visit.
I'm sorry but masks have been proved to be useless. If your facility is not up to date on basic knowledge, how can I have faith in his care provided here?
Or go my route and wear an old WWII Soviet gas mask. They will force you to remove it in which case you can say it's the only mask that can stop this virus!
